Description: Oversee and manage all aspects of community based programs and services, maintain and promote program within MN with key constituents.
Reports To: President/CEO
Position: Part Time - Full-Time
Salary Range: 28,000 -38,000
Duties/responsibilities include but are not limited to:
-Oversee and Manage all aspects of Program including employees and volunteers
-Recruit, interview, and hire or sign up volunteers and staff
-Plan and administer budgets for programs, equipment and support services.
-Act as consultants to agency staff and other community programs regarding the interpretation of program-related federal, state, and county regulations and policies.
-Complete required documentation, ensure that all time lines and policies are strictly followed and adhered to
-Direct fund raising activities and the preparation of public relations materials.
-Work with individuals with a variety of disabilities including developmentally disabled and mental health
-Responsible for assessing the medical and social needs of the client and ensuring they receive all of the necessary care and services they require.
-Provide and/or oversee case and care coordination to families, children, and adults with disabilities coordination based on assessment, including coordinating the delivery of the approved medical and social services.
-Travel throughout several counties to meet consumers in the community and/or their home
-Conduct assessment & Care Plan with clients, Act as an advocate for the client/family
-Communicate with physicians, providers and county social workers to discuss the care plan and provide updates on client progress as needed.
-Assist client with transition of care to/from institutions.
-Bachelor's Degree in Nursing or Social Work, Master's Degree Preferred. Must have a current MN license as an RN or SW; or if not, must have a four-year degree in a closely related field, three or more years of experience in home and community based services and experience working with people with disabilities.
-ATP, CAPS certifications or ability to obtain certificates within 3 months of hire
-Professional verbal and written communication skills; proficient computer skills including Microsoft Office
-Ability to take initiative, work in a fast-paced, complex and diverse situations
-Must have a valid driver's license and a reliable, insured vehicle with a clean driving record
