Job Description

Are you an experienced Case Manager who has a passion for Women’s issues? Are you detail-oriented with the ability to lead a team? A non-profit Manpower client in the Des Moines metro has an immediate need for a Case Manager.

•Provide support and leadership to residents.
•Prioritize and handle multiple case plans while providing ongoing assistance to residents.
•Develop individual case plans with residents within one week of arrival and continue to monitor resident and case plan for duration of stay.
•Must meet with residents at regular intervals, assess progress, identify barriers and modify plans as necessary.
•Provide consequences when appropriate.
•Develop motivational skills with residents while working within the knowledge of issues affecting homeless women.
•Model excellent communication skills with residents.
•Identify and provide access/referrals to assessments and treatment for substance abuse and mental health issues.
•Work in conjunction with other service providers to monitor progress including scheduling team case meetings.
•Analyze resident and other applicable data to determine new and changing needs of the target population in order to develop program components.
•Administer breath and urinalysis tests - either terminate or notify probation/parole officers and other service providers when positive test occurs.
•Fully and equitably enforce all rules and procedures of the company.
•Provide all necessary information on residents to staff for consistency.
•Monitor and provide feedback to all external partners who provide services on or offsite.
•Responsible for monitoring compliance of residents’ medications.
•Maintain strict confidentiality of client records, conversations and company information.
•Maintain HMIS ServicePoint records for all incoming and exiting residents.

•Must have a minimum of a B.S. in social work, psychology or related field and experience working with high-risk population.
•Experience in substance abuse and mental health issues preferred.
•Experience in direct counseling, program planning, supervision and coordinating community services.
•Must have conflict resolution skills, be innovative and a self-starter.
•Exceptional interpersonal and communication skills.
•Working knowledge of computers and Microsoft Office programs.
•Must be able to obtain a ServicePoint license.
•Must be sensitive and supportive of women’s issues.
